How do I make sure the USDT leaves as BEP-20?

Check the network name in your wallet before sending: it should read BNB Smart Chain, BEP-20 standard. Withdrawing from an exchange, pick that same network on the form. A transfer from another network cannot start this exchange.

Why does the amount change if it is the same dollar?

Because moving between networks is not free: the dollar stays a dollar, but the move has its own price. You lock the exact rate before you send, and it does not change after that.
